AC Joint Sprain
Overview
An acromioclavicular (AC) joint sprain occurs when the ligaments connecting the collarbone (clavicle) and shoulder blade (acromion) are overstretched or torn.
This injury often results from a direct fall onto the shoulder or collision during sports. Depending on severity, it can cause pain, swelling, and visible deformity at the top of the shoulder.
Signs & Symptoms
- Pain at the top of the shoulder, especially with cross-body or overhead movements
- Swelling or bruising over the AC joint
- “Step-off” appearance or bump at the collarbone
- Weakness or pain with lifting or pushing
- Tenderness on palpation of the joint
Causes
- Fall onto the shoulder or outstretched arm
- Contact sports (hockey, football, rugby)
- Repetitive overhead or heavy lifting
- Poor scapular control or posture increasing load on AC joint
