Spondylolisthesis

Overview

Spondylolisthesis occurs when one vertebra slips forward over the one below it, most often in the lower back. This shift can irritate spinal joints, muscles, or nerves, causing localized pain or radiating symptoms.
It can result from repetitive extension movements, degenerative changes, or congenital defects, and it commonly affects active adults, especially those engaged in sports involving spinal loading or extension (e.g. gymnastics, weightlifting, football).

Signs & Symptoms

- Localized pain in the lower back or buttocks
- Stiffness or tightness in the hamstrings
- Pain that worsens with standing, extension, or activity
- Relief when sitting or bending forward
- Possible numbness, tingling, or weakness in one or both legs if nerves are affected

Causes

- Stress fractures in the vertebral arch (pars defect)
- Degenerative changes in discs and facet joints
- Congenital or developmental alignment issues
- Overextension or repetitive hyperlordosis movements
- Weak core and hip stabilizers
